If the doctor agrees, Repeat Prescriptions may be obtained
by the following methods:-
- By using the pre-printed request slip, ticking
the items required and dropping it into the green repeat
prescription post box in the entrance hallway. If you do
not have a repeat prescription slip please ensure that your
name, address and the items you require are clearly indicated
on your written request
- by post - enclosing
a stamped addressed envelope
- by fax - 01283 791863
- by asking at
the Reception desk for a repeat prescription form to complete.
- & best of all, using this website and completing the online prescription form.
In order to avoid mistakes, repeat
prescription requests cannot be taken over the telephone.
If your repeat prescription is received by 12
noon your new prescription will be available for collection)
the NEXT DAY - from the surgery
after 9.00am or your medication from Alrewas Pharmacy
only after 5.00 pm
Alrewas Pharmacy collects
the prescriptions from the surgery and prescriptions
will always be passed to them for dispensing unless otherwise
requested. If
you wish to collect your prescription from the surgery
to enable you to take it to a pharmacy of your choice
please mark it clearly “For
collection”. See
above for times.
When on regular medication the doctor
may ask you to attend for a medication review from time
to time.
